2. Program Runs From: 8:30 AM - 1:00 PM - **Thursdays late start 9:30 am-1:00 pm
3. Maximum Capacity: 25 students per day 
4. Lunch: Continues to be free for the 23/24 school year 
5. Referrals: Grades for 7th and 12th grade students 
   • Under the influence of alcohol/drugs 
   • Possession of drug paraphernalia
   • Considerations when referral for behavioral issues: Avoid sending students​ with more severe behavior problems (e.g, history of aggression towards others, has symptoms of suicidality or self-harm, acts impulsively ​and continuously interrupts the classroom, and/or does not consider or understand consequences of his/her behavior.)
6. Students with Disabilities

• Please make sure to identify SWD students on the Referral & Consent Form Ms. Carrillo will work with the Special Education teachers and IAs in the special education classrooms/program next to hers.

TRANSPORTATION: 

It is the parents’ responsibility to transport the student, but help is available if needed. 

Determine if:

• Parent will transport 
• Student will walk to the program
• District bus - ONLY for South, Ball, Sycamore, Katella 
• Public Transportation (Daily bus passes are available)
• Please check with your site FACES or Ms. Carrillo for bus passes. Supply depends on availability. For special circumstances, contact the McKinney-Vento office. Contact: Elizabeth Lorz (lorz_e or 714-404-4170)

REQUIRED ACTIONS:

1. Referral & Consent Google Form: Pathways to Success - July 2023
   • Use this to refer and document consent for both the Pathways to Success and Tobacco Tuesday
   • Use the hard copy at your site when you are able to obtain written consent, to document verbal consent, and to use as talking points. (Place these at a centralized location at your site for future reference) 
   • You must still email the Tobacco Tuesday consent form for either verbal or written consent

• Use this guide to make changes to Aeries
• Students are being referred as an alternative to a suspension. Code before and after are important for student records and Calpads reporting

  • This is where you can verify if the student attended. Social workers will use this document to report data to the your site leadership team. Data reported will include number of students, reasons for referral, etc,.)

• Transportation Request Spreadsheet

  • Here you can verify if the transportation office has confirmed your request

• Student Reflection

  • Students will complete the reflection via a Google Doc and responses will be linked on this spreadsheet
  • This information can be used when meeting with the student
