  4. Social Security Number (if you have one)
  5. Citizenship Status
  6. Credit Card

Students applying should submit their applications via the web.  The school is equipped with a media center computer lab for those students who do not have internet access at home.  The websites for the applications for the California public schools are:

CALIFORNIA STATE UNIVERSITIES (CSU): www.calstate.edu/apply

*Filing period October 1- November 30

UNIVERSITY OF CALIFORNIA (UC): WWW.UNIVERSITYOFCALIFORNIA.EDU/APPLY

*Filing period November 1 – November 30

Those students applying to a 4-year PRIVATE school or a school out of the state of California are advised to obtain those applications via the web.  Most, if not all, have downloadable applications.  Deadlines may vary, check the deadlines online.  Cypress High will not provide those applications

For the Common Application: WWW.COMMONAPP.ORG

*Filing periods vary, this requires a Counselor Report and letters of recommendation (LOR).

Applications are self reported.  Each student will be provided a copy of their TRANSCRIPT.  They MUST use this transcript when completing their applications.  Students who complete their application via memory will likely make errors which could lead to a rescinded offer of admission.   Students should not send transcripts with their application UNLESS they are specifically requested.  The application will clearly list those items needed to complete the application.

Letter of Recommendation Forms

Dear Seniors,

Attached are PDFs of the Letter of Recommendation (LOR) Directions/Request Form and LOR Personal Data Form.

As a reminder, if you need a LOR for a college or scholarship application, you must give your recommender at least two weeks notice (not including holiday breaks).  You will need to give your recommender the LOR request form and attach all necessary supporting documents (brag sheet/resume, essay, personal insight questions, anything else you feel may be helpful).

This form can be used throughout the year.  You should download it to your computer in case you need to print up multiple forms.
