Walk on Coaches

Below you will find resources to complete your walk-on coach hiring process. Your initial point of contact should be with the school site for which you will be coaching. Human Resources will clear you for your assignment once all the steps below have been completed in a timely manner.

 

Please work on gathering the documents below to avoid any delay in your hiring process and to avoid stipend pay delay or potential pro-ration. Once you have received your intent to work form from the school please contact your designated Credential Technician. Below you will find contact information for your designated school.

 

 

District Walk-on Coach Training

  • Current and new hire walk on coaches: This training will transition to an online module. TBA

 

Employment Documents (new hires will receive an email from NeoGov to complete the new hire packet:

  • AUHSD WOC Application

  • I-9 Form (Complete Section 1) (Print Pages 1 & 2)

  • W-4 Form

  • Notice of Exclusion from CalPers Membership

  • Reciprocal Self-Certification Form

  • Demographic Data

  • Designation of Beneficiary

  • Oath Allegiance *Please leave blank, you will sign the day of your appointment

  • General Safety Rules Acknowledgement

  • Drug Free Work Place Policy

  • Personal Emergency Information Sheet

  • Information Technology Acceptable Use Agreement

  • Child Abuse Reporting Procedures

  • Choice Physician (Workers Comp Form)

 

Additional Employment Requirements:

  • Copy of a negative Mantoux TB Test taken within the last sixty (60) days. If you have worked in another school district and the TB test is valid (taken within the last four years), it will be accepted.

  • Two clear copies of two forms of personal identification (i.e.: Valid Driver License, and Social Security Card). Applicant must bring original documents to processing appointment.

  • To avoid any breaks in service, documents must be valid for the entire season (Mantoux TB results, CPR, First Aid, Concussion in Sports, Heat Illness Prevention and Sudden Cardiac Arrest certificates). It is the responsibility of employee to maintain certificates and submit copies to school site and Human Resource Services prior to expiration date.

  • Keenan Safe Schools Mandate Reporting Certificate and Sexual Misconduct: Staff-to-Student, must be done every fiscal year. New courses added: Integrated Pest Management​​​​

  • Employee must maintain a current address, email address and contact number with Human Resource Services at all times.

 

Current Coaches:

  • It is your responsibility to maintain your certifications valid for the entire season. Once you have completed the course(s) required, please scan your current certificates to your designated credential technician.
